This form is anonymous and confidential. The form will be sent to select College officials including the Title IX Coordinator, Director of Student Conduct and the Director of Security and Safety.
Completing this form does NOT constitute a police report or a student conduct report. The college will not contact you unless you provide your contact information.
Survivors or witnesses who may not initially be inclined to report an incident of sexual harassment or assault to the police or to DCC have the right to change their mind at any time.
Anonymous reports are used for the following:
Gathering and recording information regarding the incidence of sexual harassment and assault on and around our campus.
Tracking incidents of sexual assault on campus; the information on this form will be used on the annual report submitted to the Department of Education in compliance with the Clery Act.
Informing the community about the campus climate.
DCC strongly encourages all survivors of sexual harassment and assault to report the incident to the College or police, and/or to avail themselves of any/all of the resources listed as part of the DCC Sexual Harassment and Assault Prevention Program.

Reporting drug or alcohol use here will not result in any sanctions for the reporter or survivor in accordance with the College’s Policy on Drug and Alcohol Amnesty.
