If you were born on or after January 1, 1957 and are taking 6 credits or more, New York State Public Health Law requires that you provide your college with proof of immunity against measles, and mumps and rubella.

By law, if you do not provide this information within 30 days (45 days for out-of-state students) of the start of classes, you risk being dismissed from the College and are not eligible for a refund.

Please download the form at right and bring, mail or fax it to the Health Office immediately.
